ALL FAMILIES MUST FIRST BE REGISTERED WITH HHF AND THEN REGISTER FOR THIS PROGRAM TO BE ELIGIBLE FOR PARKING REIMBURSEMENT.

 

Hero Hunter Foundation is excited to announce that we will be starting a new program: Little Hero Parking Support Program- reimbursement for parking costs for all families currently undergoing treatment for pediatric cancer and blood disorders at Perth Children's Hospital.

This form is designed for all Hero Hunter Foundation families who are currently ON TREATMENT (or within 12 months since finishing treatment).

Refunds will be made to your selected bank account at the end of each month. Proof of payment can be in the form of a receipt or a bank statement clearly showing the date, location, and amount. No payments will be accepted if these are not clearly shown. All receipts must be submitted by the 25th of each month to be included in the months batch payment.

Each family will receive up to $500 p/a in parking reimbursement . Please fill in the form below at your earliest convenience and we will be in contact via email with more information once the hospital have verified that your family are eligible for this service.

Please note:

• This service is only for families currently on treatment of finished treatment within the last 12 months

• No back payment will be provided for parking prior to the program's commencement.

• Proof of payment must be submitted no later than 28 days after validation.

• Only 1 bank account can be registered per family.
